Avoca Nursing & Rehab Center is a new, 46-bed skilled nursing facility and 20-apartment assisted living complex in Avoca, Iowa, less than an hour from Omaha. Our Center provides a continuum of care to seniors in our area.
We have successfully returned 70 percent of the patients admitted for skilled care during the last year to their homes, helping them to enjoy independent, productive lifestyles.
Therapy services are provided by our own in-house department. We work with orthopedic patients and stroke rehabilitation to assist our seniors to the best possible level of health care.
Our caring staff is our strongest asset. This is evidenced by the Center’s clean, comfortable living environment and the bustling activities provided to our residents and their families. Many activities are scheduled throughout the day to keep our seniors active and independent for as long as possible.

Avoca Nursing & Rehab Center is operated by Care Initiatives, a non-for-profit organization committed to providing the highest level of quality care for Iowa’s seniors. Our Center is centrally located for families and residents in order to meet health care needs.
