Providence Home Health provides skilled in-home services to patients of all ages as a transition following hospitalization and as a cost-effective alternative to inpatient care. Under a physician's direction, Home Health can provide a variety of health care services, including:
· registered nursing
· medical social work
· home health aides
· rehabilitation therapies
Providence Hospice provides expert, compassionate care to adults and children with terminal illnesses or conditions. A typical Hospice team may include the patient, family members and chosen care givers, the attending physician, the Hospice medical director, registered nurses and home health aides, medical social workers, therapists, and chaplain. Hospice services are provided through outreach in the community.
Providence Hospice assists in physical, emotional, social and spiritual end-of-life needs, with an emphasis on effective pain control and symptom management.
Services are provided either in the home or in a long-term care setting. |
