Perham Memorial Home Care
Bringing together the health care services an individual requires to stay independent at home is the purpose of Perham Memorial Home Care. Services are provided 24 hours a day, seven days per week to persons living within a 30 mile radius of Perham.
Skilled nursing services are provided by registered nurses. These nurses provide care according to the individualized plan developed by the patient, the patient's family physician, and other professional personnel. Home health aide support is provided by certified nursing assistants. Other home health services that are available include medical social services, physical and occupational therapy, and speech therapy.
Being hospital-based allows Perham Memorial Home Care direct access to physicians, equipment and medical expertise.
