The professional staff at Life Care at Home agencies consist of registered nurses, licensed practical nurses, home health aides, physical, speech and occupational therapists, and medical social workers. All disciplines work together to ensure each patient's needs are met. All members of a patient's care giving team regularly meet to discuss the patient's progress. Each staff member also attends regular in-service training, which provides updates on care practices and procedures.
To receive Medicare benefits for home health services, patients must have a doctor's order. After obtaining this order, Life Care at Home professionals work with the patients and their families to develop a plan of care. This plan is reviewed and signed by the doctor upon development and every 60 days thereafter for as long as home care is needed. Life Care at Home meets all of your home care needs with:
