Cornerstone Home Health Programs include: *Independent Living Services-The goal of the program is to help people remain safely independent in their own homes and communities. All services and programs are supervised by a Registered Nurse *Personal Care-Agency based Personal Care & Consumer Directed Personal Care *Care Coordination-The goal of the Care Coordinator is to assists the client in identifying and obtaining the services necessary to allow the individual to remain safely independent in the least restrictive environment of their choice *Skilled Home Health Services-A "traditional" Medicare program offering nursing, home health aide and therapy services to those homebound individuals with acute or worsening medical conditions. These services require your doctors orders and oversight
