Posted: June 29, 2009 -------------------------------------------------------------------------------- Qualifications Fluency in French, basic English required; A Registered Practical Nurse possessing a license from the applicable Provincial authority and a Medication Administration Certificate. Basic Cardiac Life Support (B.C.L.S.) certification. Strong knowledge base and experience in rehabilitation and community social services. Excellent team player with strong interpersonal and communication skills. Responsible for the coordination and case management of patients. Patient care documentation. Facilitate and participate in patient/family education utilizing adult education principles. Able to respond to crisis situations with demonstrated critical thinking. Able to comply with the physical demands of the unit. Excellent organization and time management skills. Health Assessment course required, or successful completion required within six months of employment. Demonstrated ability to function as a team player. Demonstrated patient-focused care. Good communication and interpersonal skills. Second language is preferred. Preference will be given to applicants with acute care, orthopedic, Stroke Recovery, and/or rehab experience. Key Responsibilities As the Registered Practical Nurse, you will provide planned patient-oriented, goal-directed nursing care in accordance with the provincial standards of care. The RPN works within an interdisciplinary team to provide patient care. The nurse will ensure consistency in approach and optimum benefit for the patients by assessing, and identifying patient goals, planning, implementing, and evaluating the patient's progress. The RPN uses knowledge, skill and judgment to assess, plan, implement and evaluate nursing care for a convalescent/rehab patient population. The RPN functions in a leadership role to organize the patient care assignments. Regularly monitors and changes the nursing plan of care for the patient. Teaches and evaluates the learning of patients, families, nursing students.
