Individualized attention begins as soon as you are admitted, and our skilled nurses meet with you regularly to discuss your ongoing needs. Certified nursing assistants provide round-the-clock medical care or help with routine activities, and therapists provide rehabilitation to any resident who needs it.

Azalealand offers a full complement of services and accomodations to our residents and family.

Lafayette Nursing and Rehab Center accommodates 179 residents. We are Medicare certified and our center also features a 25-bed postacute rehab unit and a 24-bed dementia unit. We have RNs, LPNs and CNAs 24 hours a day. We also have physician services available seven days a week.

All levels of care are provided on the Americus campus: independent living, assisted living, skilled nursing , and Alzheimer's care.
