Heritage Manor – Colfax first opened as Octavia Manor in 1971. Octavia Manor was originally licensed as a 55-bed intermediate care facility. In 1981, five new rooms were added, along with a new dining room. Heritage Enterprises purchased Octavia Manor in July of 1995. In January 2000, Heritage Manor Colfax was approved as a skilled nursing facility and became Medicare certified in March 2000.

As a Medicare and Medicaid certified facility, we offer respite and hospice care, in addition to physical, occupational, and speech therapies. Our facility is proud of our direct involvement with our residents – we are good communicators and uphold our duties as caregivers. We also keep our residents involved in activities both on-site and within the community.

Lincoln Manor is a 140 bed licensed Intermediate and Skilled Nursing Facility. Our family owned facility was first opened in 1975 under the direction of Gershom Cohn.
