We understand that each resident is an individual with unique needs and preferences. That's why we meet with residents and family and conduct an assessment - prior to move-in and at regular intervals after move-in - to understand the need for assistance and support.
This assessment helps determine the appropriate care and service that preserve each resident's dignity, encourages independence and best enables freedom of choice. The assessment results in determining a specific service level based on the amount of assistance provided.
Upon completion of the assessment, a team comprised of the Executive Director, Assisted Living or Reminiscence Coordinator, Health Care Coordinator, and our designated Care Manager prepares an individualized service plan that is monitored on a daily basis.
Because we know that quality of life depends on both environment and relationships, we focus on these two factors. We have determined that a carefully thought out physical environment and the culture within it would come from the people and relationships living and working in that setting.
At Sunrise, we recognize that diet is one of the most important aspects of a senior’s life. All of our communities serve three, delicious and well-balanced meals daily. Plus snacks are always available throughout the day.
