State Center Nursing & Rehab Center is a 48-bed skilled nursing facility. Our pleasant, home-like health care Center is staffed by caring, professionally-trained personnel from our local community. We provide a wide range of specialized services, including:

State Center is Iowa’s Rose Capital, located along U.S. 30, the Lincoln Highway. State Center is an authentic 1930’s Main Street Community. Watson’s Grocery Store, a turn-of-the-century grocer with antique fixtures and memorabilia, was placed on the National Register for Historic Places in 1999.
