Sholom Home West, located on the Ackerberg
Campus in St. Louis Park, offers 179 beds located
in large, comfortable private and semi-private
rooms. The building is enhanced with a chapel,
library, gift shop, beauty salon/barber shop,
auditorium, courtyard and recreational spaces. It
is connected by walkways to other Alliance
continuum of care facilities situated on the
campus – Knollwood Place Apartments, the
Roitenberg Family Assisted Living Residence, and
Menorah West Apartments.
Sholom Home West is an integral part of Sholom
Community Alliance’s nearly 100-year legacy of
excellence and caring service within a Jewish
environment. One of the Alliance’s two nursing
homes, Sholom Home West is dedicated to
providing residents specialized, thoughtful
attention, helping each attain his or her optimal
level of physical, social, and psychological wellbeing.
A comprehensive, individualized care plan
is developed and implemented by an
interdisciplinary team of professionals.
Offering long-term care services, short-term
rehabilitative treatment, memory loss/dementia
care, respite and end-of-life care, Sholom Home
West also provides physical, occupational,
recreational and speech therapies to assist
residents in achieving and maintaining the
highest level of self-sufficiency possible.
Residents (along with their families) work with
licensed social workers and licensed nursing staff
to meet individualized care goals. A medical
director and consulting psychologist provide
additional support. Sholom Home West is licensed
by the State of Minnesota and certified by
Medicare and Medical Assistance.
