Crestview Nursing and Rehab Center is a 65 bed skilled nursing and rehabilitation facility conveniently located off I-80 in West Branch, Iowa, just 10 miles east from Iowa City. Our beautiful new building that opened its doors in June of 2000, boasts a gorgeous center courtyard for residents enjoyment. At Crestview, we are committed to providing personalized service, stressing the individual’s right to dignity and respect. The staff is dedicated to the highest standards of a team-oriented care program while providing compassion and understanding for our residents and their families. We were awarded the AHCA Step 1 Quality Award in 2001 and the University of Iowa College of Nursing/John A. Hartford Center of Geriatric Excellence LTC Facility Award in 2003. We are also proud of our successful rehabilitation record, having assisted many residents in their return to home or a more independent living situation. Our rehabilitation services, which are also offered on an outpatient basis, include physical, occupational, and speech therapy. Our quality assurance program works to ensure the best possible care for our residents through daily monitoring of services and immediate response to identified concerns. We understand that each resident is a special person with unique emotional, spiritual, intellectual, and physical needs.
