Clare Bridge is a community dedicated to the special needs of individuals with Alzheimer’s Disease and other forms of memory impairment. Everything about Clare Bridge is designed for one, singular purpose – an uncompromising commitment to those with dementia and their families. From the building design to carefully selected staff to specialized programming to professional training, we are focused on the individual needs of each person we serve. At Clare Bridge, residents have a sense of belonging. We protect their spirit and support their self esteem by filling their days with tasks that are familiar, purposeful and comforting.
Highly Individualized Care
At Clare Bridge, we don’t see the disease we see the total person. We understand that losses due to dementia are progressive and that residents’ needs are continually changing. Clare Bridge provides each resident with an individualized service plan to anticipate and meet their unique needs, to help them feel safe and add meaning to their day. Physical and behavioral needs, as well as life history play an integral role in maintaining familiar daily routines and service plans for each resident. These plans are reviewed regularly by staff to ensure that we meet the changing needs of our residents.
Compassionate Assistance
Individuals working with memory loss must have compassion, understanding and creativity. Staff is carefully selected and trained to understand and manage the unique challenges associated with dementia. Many of the same caregivers are with a resident throughout the day, helping with all their needs. Consistency of staff is maintained in order to build familiarity and trust. Dementia care experts work at Brookdale Senior Living to provide training, ongoing support and creative problem solving to all of its’ memory care staff.
Creating Familiarity in Daily Living
Clare Bridge has lovely, peaceful surroundings inside and out. Our home-like setting was designed to re-create environments people have enjoyed throughout their lives. Interior spaces are scaled and decorated like those of a private home. Indoor and enclosed outdoor walking paths give residents the opportunity to explore safely. Residents may have their own rooms and decorate with personal furnishings and other items that help connect them to their past.
At Clare Bridge we believe that every life story is important, it defines us as individuals. Detailed life history information - past jobs and life roles, hobbies, family dynamics, habits and personal likes and dislikes - is embraced and integrated into residents’ daily routines. Dedicated staff offers programs and activity around the clock to meet residents’ at their point of need. Throughout the day it is not uncommon to see residents at Clare Bridge participating in aromatherapy, hand massages, current events, baking, poetry club, bird watchers club, memory lane reminiscing group and yes, some communities even have their own band. Through these specialized programs, we support and encourage each resident to participate at their highest level of capability.
