The residential program has been part of the PresbyHomes & Services family of communities for older adults since 1981.

Phoebe Ministries' two assisted living communities offer residents some support with the activities of daily living such as bathing, dressing, taking medications, or help with house management responsibilities such as meal preparation or housekeeping. Should residents need skilled nursing care, it is provided in the adjacent nursing home.

Autumn Grove Care Center is a 103 bed skilled nursing facility located in Northern Butler County. The facility is fully licensed by the state of Pennsylvania, and is Medicare/Medicaid/HMO approved. The Administrator and Assistant Administrator have also obtained their Nursing Home Administrator's license from the state.
