With four levels of living, residents may choose to enjoy a one-bedroom apartment in Independent Living; a studio with all meals and housekeeping in Residential Living; assistance with daily activities and medications in Personal Care; or skilled nursing in the Health Care Center. We are a 81 bed Skilled Nursing Facility designed to meet both short and long-term care needs. We participate in Medicare/Medicaid programs and contract with several HMOs.

Evergreen Healthcare Centers of Vallejo -- Springs Road (Evergreen at Springs Road, L.L.C.) provides a continuum of care that includes professional nursing care, rehabilitative therapy and support services to residents enabling them to maintain their independence. We are a 65 bed Skilled Nursing Facility designed to meet both short and long-term care needs. We participate in Medicare/Medicaid programs and contract with several HMOs.

Our rooms and spacious day room area are designed to provide personal privacy as well as comfortable areas to visit with family and friends.

Intercommunity Care Center is a 147 bed Skilled Nursing Facility which specializes in providing care to the confused, wandering, geriatric person. The facility provides a secured environment. The main entrance doors and perimeter fences are locked permitting residents to wander throughout the facility and its outside walkways and patios, while maintaining as safe an environment as possible.
