Resthaven is licensed by the State of Illinois Department of Public Health for 50 beds, four private rooms and 46 semi-private beds. Evergreen Nursing & Rehabilitation Center is a 120-bed skilled nursing facility, certified by Medicare and Medicaid. We are a member of the Illinois Healthcare Association and are certified as an Exceptional Care Facility by the Illinois Department of Health & Family Services.
