Evergreen Nursing & Rehabilitation Center is a 120-bed skilled nursing facility, certified by Medicare and Medicaid. We are a member of the Illinois Healthcare Association and are certified as an Exceptional Care Facility by the Illinois Department of Health & Family Services.

At Graham Hospital, we have a proud and rich history of serving the healthcare needs of Canton and the surrounding communities. In 1909, the original structure of the hospital was given as a gift to the community by Alice and Caroline Graham. Since then, the hospital has grown to be the progressive, comprehensive medical facility it is today.
