Evergreen Mountain View Health and Rehabilitation Center (Evergreen at Mountain View, L.L.C.) provides a continuum of care that includes professional nursing care, rehabilitative therapy, dementia focused care and support services to residents enabling them to maintain their independence. We are a 146 bed Skilled Nursing Facility designed to meet both short and long-term care needs. We participate in Medicare/Medicaid programs and contract with several HMOs.
