Evergreen Butte Health and Rehabilitation Center (Evergreen at Butte, L.L.C.) provides a continuum of care that includes professional nursing care, rehabilitative therapy and support services to residents enabling them to maintain their independence. We are a 186 bed Skilled Nursing Facility designed to meet both short and long-term care needs. We participate in Medicare/Medicaid programs and contract with several HMOs.

Situated high on the western edge of the Continental Divide, at the foot of the Highlands Range, the 11-acre Waterford on Elizabeth Warren commands a grand view of Our Lady of the Rockies as well as sweeping vistas of open fields and wooded hills.
