Farmington Square of Medford

Farmington Square of Medford offers Assisted Care, Enhanced Care, the newly opened Transitions™ program and Memory care. One assisted/enhanced care building is for those who are cognitive and need help with some activities of daily living. The enhanced care building is a home for people who are cognitive but less ambulatory and may need two-person assists or transfers from wheelchairs. The Memory Care buildings are tailored to meet the needs of those who are in more advanced stages of Alzheimer's. One houses 18 residents and one houses 16 residents. We are proud to offer the newly-developed Transitions™ program that is designed to meet the ever-changing personal care needs of those displaying the initial signs of memory loss. Our program of Progressive Care is also available in the Transitions building.
