There are three distinct levels of care within the Lutheran Home’s memory loss continuum – each designed to meet both the physical and cognitive needs of those with Alzheimer’s and other Dementias. Compassionate Long-term nursing care is also available for those in need of twenty-four hour care. The Lutheran Home offers four distinctive and separate levels of nursing care to meet each individual’s needs. Medicare certified skilled nursing and rehabilitation therapies are available on both an inpatient and outpatient basis. Specialized care includes orthopedic surgical recovery, neurological care, pain management and wound care.

The Lutheran Home also offers services to those living in the community. Lutheran Home Health Services, a licensed home health agency, provides care for individuals in their home. Cherished Place and Connections are both Adult Day Care programs designed to provide daytime structure for those still living at home. Cherished Place provides a Memory Support program and is specially designed for people in all stages of Alzheimer’s disease and other Dementias. Connections is designed for older adults who are looking for social interaction during the day.
